Do repeat patients get tired of a clinic's scent? A patient visiting monthly has weeks between exposures and is unlikely to habituate. The people at risk are staff who are in the room forty hours a week, and their habituation is why the level tends to rise.

How does the level creep upward? Staff stop perceiving a constant background, conclude the machine is not working, and turn it up. Nobody turns it back down, and the next increase starts from the new baseline.

Commercial scenting goes wrong in a predictable order: somebody buys a machine to cover a smell that has a source, puts it where the socket is, runs it at full intensity because day one felt weak, and ends up with a room people comment on for the wrong reason. How do you prevent fragrance fatigue with patients who return monthly for a year?
By protecting the level from your own staff, because they are the ones who habituate and the patient is not. Habituation to a constant background smell happens within days of continuous exposure.
