The thank-you gifts I remember handling well were all small and all arrived with something written. The ones that went wrong were larger and arrived with a card. I do not think that is a coincidence. A large object from a junior person puts the recipient in the position of having to respond in kind or explain why they are not, and neither is a nice position to be in when the relationship was, until that moment, uncomplicated.
What I would spend the effort on instead is the four sentences. Name the advice, say roughly when, say what you did, say what happened. I know that sounds too simple to be the answer. Try writing it and you will find it is the hardest part of the whole exercise, which is a fair sign that it is also the valuable part. Nobody can outsource it and nobody can buy it.
