I have walked into a good many freshly renovated spine clinics that did not feel premium and a good many older ones that did, and the difference was never the stone. It was order, light, sound and smell, in that sequence, and none of them needs a builder. Order comes first because a patient reads it before anything else: a desk with nothing on it but what the desk is for, a queue that is visibly managed, paper that matches - one typeface on the sign, the form and the prescription pad - chairs that are clean at the seams and set so that the slowest walker has the nearest one, and a room that smells of nothing wrong. That last item is part of order, not part of scent. Disinfectant residue, monsoon damp, dead air-conditioned air and the occupancy note that builds in a rehab room are all faults of order and are all fixed by ventilation, cleaning timing and laundry, never by fragrance; what a pain clinic should fix first is the sequence. If there is a window, let it work; a reception lit partly by daylight reads as a better-kept building than any fit-out.
