Almost every founder starts this conversation at step seven and works backwards, which is why the answer never quite holds. Here is the whole method in the order it has to be done, from a drawing with no walls up on site yet to the day you finally decide how strong anything is.

Straight answer

In a fixed order, and the order is the whole answer. Step one is ventilation, and it is not a fragrance decision at all - it belongs to your architect and your services engineer. Where the fresh air comes in, how many air changes each area gets, where the supply and return grilles sit, whether the clinical wing is on its own loop, whether the chemical store and the washrooms have their own extract. Everything after this is downstream of it, because air movement decides where any smell in the building goes, yours and everybody else's. Get this right and half of what founders later call a fragrance problem never appears. Step two is the cleaning protocol, also not a fragrance decision. Which agents, at what dilution, on what rota, stored where. Heavy cleaning scheduled outside patient hours, real air changes afterwards, containers closed, cloths and mop heads dried in a ventilated store outside any public area. Fragrance is never a mask, never a substitute for cleaning or ventilation, and the agents themselves are your decision as the clinician and your facilities team's, never mine. If the cleaning protocol is wrong, no fragrance will rescue it and adding one makes a third smell that belongs to neither. Step three is the fragrance-free zone map, and it is written before the scented one. Take the room schedule and cross off: examination rooms, scan and imaging rooms, procedure and recovery rooms, prenatal and postpartum care rooms, lactation and feeding rooms, pelvic-floor and other treatment cabins, sample collection, changing rooms, staff areas and stores. In a typical integrated centre that removes more than half the plate. The reason is the same for every room on the list and needs no other support: she is undressed or lying down, in a room she cannot leave, for a period she does not control, and she did not choose the fragrance. Consultation rooms that do no examination take a faint drift through the door and never a device of their own. What remains - entrance, reception, waiting, corridors, and any genuinely non-clinical lounge - is the scented plan. Step four is the consent architecture, and it is designed rather than improvised: at least one seating area kept effectively unscented by design, positioned out of the machine's air path at the drawing stage and known to the front desk; any member of staff able to switch the machine off in seconds without finding a manager; and a standing instruction that if a patient objects the answer is to reduce or switch off, never to defend it, never to ask her to sit with it, never to explain why she should not mind. This belongs to your architect and services engineer, not to me, and it is the single highest-leverage thing on the page. Ask for fresh-air provision and air changes per area, a clinical wing that is not fed by the public return, dedicated extract for washrooms and the chemical store, and a drawing that shows every supply and return grille. Then keep that drawing, because step six is decided from it. A centre with sound ventilation rarely has a fragrance problem; a centre without it has one that no machine can fix, and buying a bigger machine simply distributes the problem more efficiently.
Tip: Ask for the AC layout as a separate sheet. It is the sheet the scenting plan is drawn on.

Step two - write the cleaning protocol before you write anything about scent
Sequenced, not layered - and never masked
Agents, dilution, rota, storage. Measure dilution rather than pouring by eye, because over-strength mixes leave residue that keeps releasing vapour for an hour. Put heavy cleaning before opening and after closing, ventilate genuinely afterwards, and keep concentrates, cloths and mop heads closed and stored outside the public zone with their own extract. Then set the machine's timer to start after the airing window, so the two are never running at once. Fragrance never covers, replaces or stands in for cleaning, ventilation or infection control, and which agents you use is entirely your decision as the clinician and your facilities team's.
Tip: Put a measuring jug in the store cupboard. It changes a building faster than anything I sell.

Step three - draw the fragrance-free map before the scented one
In a real integrated centre this removes more than half the plate
Go through the room schedule and cross off every room where a patient is undressed, lying down, mid-treatment, connected to equipment, feeding, giving a sample or waiting on a result. Examination, scan and imaging, procedure and recovery, prenatal and postpartum care, lactation and feeding rooms, pelvic-floor and treatment cabins, sample collection, changing rooms, staff areas and stores - all unscented, always, with no reduced version and nothing brought in from home. Consultation rooms that do no examination take a drift through the door and no device. What is left is your scented plan, and it is usually a third of what the founder expected.

This is the message I most want to receive and the one that arrives least often, because it comes from someone who has not built anything yet. Almost everyone else writes to me at the end, with a finished building and a question about fragrance, and by then four of the seven decisions have already been made by accident. When a founder asks me at drawing stage, the honest thing I can offer is an order rather than a product.

The order matters because each step makes the next one answerable. You cannot choose a placement without a ventilation drawing. You cannot choose an intensity without knowing which doorway you are protecting. You cannot decide a register sensibly until you know how much of the building is even in the plan, and in an integrated centre it is less than half. And the two steps that do the most - the ventilation and the cleaning protocol - are not mine to sell you, which is exactly why they are first. If those two are right, the fragrance layer is a small, cheap, pleasant last decision. If they are wrong, no machine in my catalogue will help, and I would rather tell you that now than after you have paid me.
