For a family that has travelled four hours, the washroom is the first real test of whether a practice is run properly, and it is the most common source of a waiting-room odour complaint. The thing that stays with me about Nagpur clinics is the journey. A family leaves a town in Vidarbha at five in the morning, travels three or four hours with somebody who is unwell, and arrives into a waiting room at ten. What they need in the first ten minutes is water, somewhere to sit together, a washroom that works, and a room that is not too hot. Those come a long way before anything I sell.

And I want to be careful here in a way I am not in a showroom. Ambient fragrance has no clinical benefit, and I will not let it be described to a patient as though it does. Some of the people in that room are nauseated, pregnant, mid-treatment or migraine-prone, and for them a scented waiting area is not a nice touch.
