Because the building has had no air changes for about fourteen hours, and every material in it has been quietly releasing something the whole time. This is arithmetic rather than hygiene. A treatment table is a vinyl skin over a foam core, and both release slowly and permanently. So do MDF joinery, laminate edges, carpet backing, chair foam, the adhesive under a vinyl floor, laundered covers stacked in a closed cupboard, the contents of a bin, and the residue of whatever the surfaces were wiped with at closing. During the working day none of that reaches a concentration you notice, because doors open, people move, the air conditioning cycles and the volume is being disturbed constantly. Overnight the rate does not change - the removal does. Fourteen hours of slow release into a sealed volume with nothing leaving it produces the total you walk into at eight in the morning. That is what stale is. Three things worth understanding before you change anything: what the building is actually giving off, why fifteen minutes is the number, and where the fragrance source belongs in the sequence.

The inventory
Everything in a clinic releases something, slowly and forever
A treatment table is vinyl over foam and both give off a low, steady stream of volatiles for years. So do the chairs, the joinery, the laminate worktop, the carpet backing, the flooring adhesive and the paint. Add the textiles: laundered face-cradle covers folded into a closed cupboard, towels, the laundry bag waiting for collection. Add the consumables: hand wash at the basin, the gel on the counter, the wipe-down solution used at closing. Not one of these is a fault. Every occupied building does this. The reason you notice it at eight in the morning and not at three in the afternoon is that at three in the afternoon it is being removed as fast as it appears.
Tip: Open the linen cupboard and the under-sink cabinet during the purge. Sealed cabinets are small concentrated volumes that empty into the room the first time somebody opens a door.

The purge
Fifteen minutes, both ends of the building, doors propped
You are trying to exchange the air, not stir it. That means a path: an opening at one end, an opening at another, and the internal doors held open so the building is one connected volume rather than a set of pockets. A small clinic with cross-ventilation will turn its air over several times in fifteen minutes. A clinic with one window at one end will do considerably less, and a sealed clinic with no operable opening at all cannot do it by opening anything - that reader needs the fresh-air side of the air handling, which is a conversation with a facilities engineer rather than with me. Run it before the cleaning, not after, if your cleaning happens in the morning.
Tip: Fifteen minutes is the standard because it fits between unlocking and the first client. Twenty is better. Five is stirring, not exchanging.
Switch any source off during the purge. A running Vaayu during an air exchange is just fragrance going out of the window, and it also makes the room harder to judge.

The sequence
Purge, settle, then scent - and never overnight
Once the air has been exchanged, close the building, let the temperature come back and then bring the fragrance source on. In that order the scent is landing in a clean, empty room and doing the only job it has, which is a low, consistent character in the front of house. In the reverse order it is landing on top of fourteen hours of accumulation and you get both at once. This is also the argument against leaving a reed diffuser running through the night in a small sealed reception: it is a continuous source with no switch, so it contributes to the overnight total and then greets you with the result. Carrying the bottle into a cupboard at closing is a workaround. A scheduled machine is the tidy version of the same idea.

I get this enquiry most often in the first week of a new diffuser, which tells you something. Somebody buys a bottle, likes the room at four in the afternoon, and then arrives at eight the next morning to a version of the same room that seems worse than before they started. Nothing has gone wrong with the fragrance. They have simply added a slow continuous source to a building that already had several, and then shut the door on all of them for fourteen hours.
The reason I keep pushing the fifteen-minute purge is that it is the only lever on this page that costs nothing and works every single day. It is also the least popular thing I say, because it means somebody arrives earlier, and an earlier arrival is a real cost in a small practice. How long does the morning purge actually need to be?
Fifteen minutes with openings at both ends of the building and the internal doors propped. That is usually two or three full air exchanges in a small clinic. Twenty minutes is better if your first appointment allows it. Five minutes stirs the air without replacing much of it, and closing everything again after two minutes is worse than not starting.
Should I leave a reed diffuser running overnight in reception?
You can, but understand what it does. A reed has no off switch, so it runs for the fourteen hours nobody is there and its output accumulates with everything else. If your mornings are the problem, carry the bottle into a cupboard at closing for a fortnight and see whether the eight o'clock room reads differently. That test is free.
My clinic has no windows that open at all. What then?
Then you cannot purge by opening anything, and the exchange has to come from the air handling. Ask your facilities engineer whether the system has a fresh-air intake and whether it is open, because a good number of split installations have none. Until that is answered, a scent machine is not solving the thing you asked about.
Is cleaning at night making the morning worse?
It can be a large part of it. Cleaning volatiles are at their strongest in the first half hour and then decline, so a room wiped down at closing sits with them all night in still air. Moving the wipe-down to early morning, ahead of the purge, changes the eight o'clock impression more than any product will.
If the smell survives a proper purge, what does that mean?
That you have a source rather than an accumulation, and it needs finding rather than diluting. A room that exchanges its air two or three times and still reads the same has something in it that is emitting continuously - a trap that has dried out, damp behind a fitting, a bin, a cupboard.
