Grout and silicone. Porous grout and old sealant hold moisture and grime that a surface wipe does not lift. They can smell musty even when the tiles look spotless.
Damp fabrics. A towel, bath mat or loofah that never fully dries turns musty fast. Dry or wash them and the smell often lifts with them.
Poor ventilation. If air does not move, humid, stale air settles and smells off regardless of how clean the surfaces are. Run the fan or open a window.
