There are four places in a pain-clinic reception where a scent machine always ends up, and one place where it should be. The four are where the socket is, where the return grille is, where the desk is and where the corridor starts. That is the whole answer, and almost nobody's machine is there, because a machine goes where it is easy to put and the easy places are the four wrong ones. Take them in turn. By the door. There is usually a socket beside the entrance and a console table under it, and that is where the machine or the bottle lands. The door is a draught; every opening exchanges the air beside it with the street, so the output is either lost or, when a patient pauses in the doorway, breathed straight from the source. Fragrance begins inside the door, not at it makes that case in full. By the AC return. The return grille is the room's drain: air goes into it, through the plant and back out at the supply. A source near the return puts its fragrance into the duct before it has reached a single chair, and where the duct delivers it depends on what else the plant serves - if the return also serves a corridor toward the procedure rooms, the fragrance arrives exactly where it must never be. On the desk at nose height. A machine or a bottle on the patient's side of the reception counter is at the nose of every person who leans in to sign a form or hand over a report, for the twenty seconds to two minutes they stand there. Wrong place one - by the door
Where the socket and the console are, and where the draught is
The entrance has a socket for the sign, a console table for the visitors' book, and a wall that nobody sits against, so the machine goes there. Then the door opens two hundred times a day, each opening takes the output with it, and each patient who pauses in the doorway breathes straight from the source. The door position gives nothing to the room and a blast to the person standing in it, and the desk, noticing nothing, turns the machine up. The correction is not a different setting but a different wall. Should fragrance begin at the entrance or only inside reception is the full argument; the short form is that the door is where fragrance is met and never where it is made.
Test: if the scent is stronger in the open doorway than at the chairs, the source is on the wrong wall.

Wrong place two - by the AC return
The room's drain: the fragrance leaves before it has reached a chair, and may come back somewhere it should not
Air in the room moves from the supply grilles toward the return, and a source near the return puts its output straight into the duct. The seating receives almost nothing, so the level is turned up, and whatever the duct carries goes wherever that air-handling unit serves; if the return serves reception and a corridor toward the clinical rooms, or a shared plant serves more than one zone, fragrance can arrive in air that must smell of nothing. That is a placement failure and cannot be fixed with a setting. Put the source on the wall furthest from the return. Where the ducts serve more than one zone, the HVAC mount is chosen only if the duct serves the patient-facing zone alone, and the facilities engineer has the last word.
Test: hold a tissue to each grille. The one that holds it is the return. The source goes across the room from it.

Wrong place three - on the desk at nose height
The one position where every patient breathes directly from the source
The reception desk is where a bottle looks decorative and where a small machine has a socket, so both end up there - on the patient's side of the counter, at exactly the height of the nose of a person leaning in to sign. While a patient stands at the desk they are breathing from the source rather than from the room, and any level that is right across the seating is far too much at that distance. The correction for a reed is the staff end of the counter, raised on a shelf if the desk is short, away from the signing side and the chairs; the first ninety seconds - door, desk, chair has the desk as its second point. For a machine the correction is off the desk altogether and onto the wall, which is what a Vaayu's mount is for. The desk keeps one job in the scenting plan: the off switch, pressed the moment any patient objects.

Wrong place four - the corridor mouth
A passage turns a trace into a scent, and it leads toward the procedure door
The opening of the corridor to the consultation cabins is the most tempting wrong place, because it seems to solve two problems at once: the cabins will "get some", and the waiting room will too. A corridor is a narrow volume with moving air, where a trace concentrates into a scent for the patient walking it slowly. Worse, the corridor leads to the procedure door, and a source at its mouth is upstream of the one room in the clinic that must smell of nothing. Procedure rooms, injection suites, fluoroscopy rooms and recovery bays stay unscented, and the way that is kept true is by placing the source on the far side of reception from the corridor and closing doors, never by a lower setting inside. The cabin is the doctor's call and inherits a trace through its open door from a properly placed reception source; nothing is placed in the passage.
Rule: nothing in a corridor, ever. If the cabins need a trace, it comes from reception through an open door; if the doctor wants none, the door closes.

I have been sent hundreds of photographs of scent machines in clinic receptions, and I can count on one hand the ones that were on the right wall. The rest were by the door, because that is where the socket was; under the return grille, because that wall was empty; on the desk, because it looked nice; or at the start of the corridor, because somebody wanted the cabins to have some. Every one of those positions makes the machine work harder and the room smell worse.

The right wall is found on a plan, not in the room. Mark the entrance, the procedure door, the supply grilles and the return, and look at what is left: the wall the patients face, with the whole room between it and both doors. Put the source high on that wall and the entrance becomes the faintest point, the seating gets an even trace, and the procedure door has the width of reception on its side - which is the only way I know to keep that room smelling of nothing without ever touching a setting inside it. The cabin is yours.
