For as long as the door stays open, the volume of outside air behind it is, for practical purposes, unlimited compared with the café's own air, so the room's air is being replaced by street air at a steady rate the whole time the door is open, not just at the moment someone walks through.
3. Turning up intensity does not fix an exchange-rate problem. A machine set higher simply puts more fragrance into a room that is losing it just as fast, or faster — it changes how much is being spent, not how much is staying behind.

Why an open door defeats a fragrance before it starts
A fragrance doesn't fill a room the instant it's released — it accumulates, molecule by molecule, until the concentration in that air reaches a level a nose can register. That accumulation depends entirely on the air staying more or less where it is long enough for the level to build. A closed room, or a room with only the ordinary, incidental air movement of people walking in and out, holds its air well enough for that build-up to happen. A room with a door standing open the whole service does not, because the air isn't staying in the room at all — it's being continuously traded for the air outside, and whatever the diffuser adds gets carried out with it almost as fast as it arrives.
This is worth separating clearly from the idea of a "leak" or a draught, because the two behave quite differently. A draught is intermittent — a door opens, some air exchanges, the door closes, and the room's air has a chance to recover and rebuild before the next opening. An open door held open for hours is not that. It's a continuous, one-way exchange for as long as it stays open, closer to the room having no fourth wall than to the room having a slightly leaky one. The distinction matters because the fix for a draught — a bit more fragrance, a bit more frequent refill — genuinely doesn't translate to the fix for a continuously open boundary, which has no fragrance-side fix at all.

What actually holds a register, in order
Never treat a fragrance that seems to have faded as a sign the product is too weak before checking whether the door to that room has actually been standing open — this single check resolves more "the diffuser stopped working" complaints than any change to the product itself.
Sometimes the fix is simply behavioural — a self-closing hinge, a habit of shutting the door between customers rather than propping it, or moving a diffuser from a doorway zone to a genuinely enclosed washroom instead, where the boundary is dependable.
Usually, once a real boundary is confirmed rather than assumed, a modest reed diffuser or ultrasonic machine sized correctly for that room's square footage holds a level perfectly well — the product was rarely the problem to begin with.
