Should a diagnostic centre smell fresh, woody, green or neutral? It depends on what the visitor is doing at that point in the building. Somebody standing at a counter for two minutes can take a brighter, fresher air without tiring of it. Somebody seated for an hour needs something quieter, drier and more structured, because exposure adds up. Somebody cannulated, undressed or waiting in a cubicle gets nothing at all. So one building can honestly want a fresh note at the desk, a dry one in the seating and zero behind the corridor - one register, three levels.
Is completely neutral a legitimate answer? Yes, and in this category it is a stronger answer than in any other I write about. A diagnostic centre is cleaned several times a day, and that cleaning smell is part of what a customer handing over a sample came for rather than a fault to be papered over. A room that smells of a well-run building and nothing else is a perfectly good outcome. It also costs nothing, which is worth saying out loud on a page that sells diffusers.

Ask what the visitor is doing, and the four-way argument settles itself. Fragrance exposure is a function of time as much as concentration, so the same air that reads as pleasant to somebody standing at a counter for two minutes reads as too much to somebody who has been sitting in it for an hour, and reads as unacceptable to somebody lying down with a needle in their arm. That is the tie-breaker, and it is better than anybody's taste. Where people stand and move - the entrance, the counter, the payment desk - a brighter, fresher air is fine. Completely neutral wins more often in this category than in any other I write for. Your building is cleaned several times a day, visibly and deliberately, and the smell of that cleaning is part of what a customer about to hand over a sample came for. Neutral here does not mean an odourless room; it means the cleaning baseline, unaccompanied. If that baseline is a decent citrus or pine-based product rather than a heavy phenolic one, and your ventilation is working, the room is already saying what you want it to say. Changing the cleaning product costs less than a bottle and does more. Two more things. Nothing gourmand, at any dose, in any of the three levels - the morning queue has not eaten since the night before, which is a courtesy judgement about register and not a claim about appetite, hunger or comfort. One
The dwell test: how many minutes does a visitor breathe it
Exposure is concentration multiplied by time, and most scenting advice only talks about the first half. A bright citrus in a room somebody passes through is lively; the same bright citrus in a room somebody sits in for fifty minutes waiting for an ultrasound slot becomes wearing, and the person who notices first is usually your own receptionist rather than a patient. So measure dwell before you choose. If your average visitor is standing for four minutes, fresh is fine. If they are seated for forty, go drier and quieter. If you have both in one connected volume, design for the seated person, because they are the one accumulating minutes and they are the one who will mention it.
Tip: Your receptionist and your phlebotomist are exposed for a twelve-hour shift. Whatever the level is, it has to be right for them first.
Adjustable intensity is the honest reason to spend here: the Vaayu lets a long-dwell room run lower than a busy counter without changing the scent.
The baseline test: what does the room smell of already
This is the test that makes neutral a serious contender in a diagnostic centre and nowhere else. Walk the public floor twenty minutes after the mid-morning clean and be honest about what is there. If the answer is a decent citrus or pine-based product on clean surfaces with working ventilation, your baseline is already doing the job people buy fragrance for, and the customer is not offended by it - they are reassured by it, because visible cleaning is part of what they came for. Adding a second volatile set on top of a strong one does not blend them. They interfere, and you get a third character nobody chose. If the baseline is a heavy phenolic that lingers, change the product before you change anything else, because it runs three times a day and a diffuser runs once.
Tip: Do this test at three separate times of day. A centre that smells like three different buildings has a rota problem rather than a fragrance problem.

Standing, seated and supine are three different relationships with a room, and they justify three different levels of the same scent. Standing at a counter, a person is transacting and about to move on. Seated in a waiting area, they are stationary, bored and much more attentive to the environment. Behind the corridor, a person who is undressed, cannulated or enclosed in equipment is in a situation where an added smell they did not choose is an imposition, and where the room is somebody else's controlled space anyway. That is why the third level is zero rather than low. It is a flat rule and it is the part of this page I would not negotiate on, because the reasoning is jurisdictional rather than technical and it does not soften with a smaller bottle.
Tip: If a room has a curtain, a gown or a chair with an armrest for cannulation, it is on the zero list. That covers most edge cases.

I try to move the conversation off preference within the first two minutes, because a centre is not choosing a scent for itself, it is choosing one for a person who will breathe it while sitting still and slightly bored.
Dwell time is the number nobody has to hand and everybody could get in a day. Not as two fragrances, no. If the counter and the seating share one volume of air, two scents meet in the middle and the result reads as confused rather than as two zones. In a long-dwell waiting area the green and dry-woody end wears better, which is why I recommend it for seating.
Which direction should an imaging waiting room take?
The quiet, dry one. Imaging appointments run long, so an imaging waiting room usually has the highest exposure minutes per visit of any room in a diagnostic building, and the level should come down accordingly. Does a stronger fragrance ever solve a long-dwell problem?
It reliably makes it worse. If people are noticing the fragrance an hour in, the answer is less of it, not a different one and certainly not more.
