How do I find the source of a persistent clinic odour before adding fragrance?
By treating it as a search with a route, rather than as something you keep noticing and keep meaning to deal with. A persistent smell is not spread evenly through a building; it has a place it is coming from, and there are only so many places it can be. The procedure takes one early morning. Set the conditions first. Come in before anybody else, before the cleaning, and before the air conditioning is switched on, and give yourself a nose that has been outside for at least ten minutes - after twenty minutes in a building your own sense has adapted and you are no longer measuring anything. Keep every internal door shut so that each room stands as its own volume overnight; a building with the doors open averages itself, and an average is what you have been smelling for months without learning anything from it. Then the route, one room at a time. In each room: five surfaces, with your face within a hand's width of each rather than standing upright in the middle - the floor-to-wall junction, the underside of upholstery and the foam under any split seam, the inside of a closed cupboard, stored textiles, and the bin. Then one drain: the floor trap, the basin, and the condensate line from the air conditioning. Then one return vent, which is the most informative point in any room, because a return grille samples the whole volume - a smell that is strongest there is being carried from elsewhere, while a smell that is strongest at the supply side is coming from inside the unit. Write one line per room as you go, because you will not remember the third room by the time you reach the seventh. The conditions
Early, cold, closed, and before anything has been switched on
Four requirements, and each exists because of a specific way this search goes wrong. Early, because you want the building as it was left rather than as the day has made it. Cold, meaning a nose that has been outside for ten minutes, because adaptation takes a couple of minutes and it is total - after that you are the least reliable person in the building. Closed, because doors left open let the rooms average each other overnight, and an average tells you nothing about where the smell lives. And before the cooling starts, because a fan moves the evidence and can carry a smell from the room it belongs to into the one you are standing in.
Tip: Ten seconds per room, then move. First impressions are the only unadapted reading you get, and the second minute in a room is already less useful than the first.

The route
Five surfaces, one drain, one return vent, per room
The five surfaces are the floor-to-wall junction, the underside of upholstery and any split seam, the inside of a closed cupboard, stored textiles, and the bin - all read at close range, because a source that is obvious with your face near it can be undetectable standing up two metres away. The drain matters more than owners expect: a trap that has dried out in a rarely used room is an open path to the drainage stack, and it is the most common resolution I hear back. The return vent is your aggregator. Strongest at the return means the room is carrying something from elsewhere; strongest at the supply means the unit needs a service. Between them these seven checks cover almost every persistent clinic odour I have been sent.
Tip: Pour a jug of water down every trap that is rarely used - the spare loo, the store, the floor drain in an unused cabin - once a week. It costs nothing and it prevents the most common cause outright.

The record
One line per room, written in the room, twice
Write in the room. Not afterwards at the desk, because by the seventh room the third one has gone. One line is enough: the room, the strongest thing in it, and where you had your face when you noticed. Description beats judgement - "sour, strongest at the skirting behind the plinth" is worth having; "a bit off" is not. Then run it again on a second morning, ideally in different weather, because one morning is a sample and two is a pattern. If the same room and surface come up twice, you have found it. If the two readings disagree completely, the smell is often arriving from outside the building or from a neighbour, which is a different conversation and still not a fragrance one.

I do this walk in every commercial space before I quote for it, and it has cost me more sales than anything else in the business. It has also produced the two or three conversations I am proudest of, which usually run along the lines of: there is a trap behind the store room that has been dry since the shop fitters left, pour water down it every Monday, and call me in a month if you still want a diffuser. Most of them do not call.
The part people find hardest is the cold nose, because it means the search cannot be squeezed into a normal working day. Frequently asked questions
How early do I need to be in the building for this to work?
Before the cleaning, before the air conditioning and before the first member of staff, which in most clinics means the first thirty minutes after unlocking. What matters is that the building is still as you left it and that you have been outside for at least ten minutes beforehand. If you have already been inside for twenty minutes, postpone it to another day rather than doing it badly.
Why do the internal doors need to stay shut overnight?
So that each room stands as its own volume. With the doors open, a building averages itself while nobody is there, and an average has no address - it tells you the clinic smells and nothing about where. Closed doors let you compare one room against another, which is the point of walking rather than sniffing.
Why does an unused drain start to smell, and what stops it?
A trap holds a small amount of water that seals the room from the drainage stack. In a rarely used floor drain or basin that water evaporates, particularly under air conditioning, and the seal goes with it. A jug of water down each unused trap once a week keeps it sealed. It is free, it takes a minute, and it resolves a surprising share of these cases.
Should the air conditioning be running during the walk?
Off, for the first pass. A fan moves the evidence and can carry something from the room it belongs to into the room you are standing in. Once you have walked the building cold, switch the system on and walk it again - if a smell appears or strengthens at that point, you have learned that the source is in or near the equipment rather than in the room.
I ran the walk twice and found nothing. What now?
That is a useful result rather than a failure. It usually means there is no point source and you are dealing with accumulation - too few air changes, an occupancy curve across the day, or a building that seals itself overnight. Those are ventilation and scheduling questions, and a fragrance is a reasonable thing to consider once they are settled.
