Nobody regrets planning this in. A surprising number of founders regret the extension lead running along a skirting board in a reception they spent twenty lakh on, and by then the only fixes are expensive or ugly.

Should the designer plan it in? Yes, and specifically at the services drawing stage - the same round where power, data, lighting circuits and AC grilles are being coordinated. That is when a scent machine costs nothing to accommodate. At handover it costs a surface-mounted socket, a visible cable, or a position chosen because it was the only one available. Ask for it at roughly a metre off the floor at the chosen position, ideally concealed within joinery or behind a console. If the machine is going to be wall mounted, ask for the socket and any fixing to be coordinated together so the bracket is not going up over finished plaster later.

Define the position by the AC, not by the composition
Designers place objects visually, and air does not care
Left to the scheme, a scent machine ends up in the corner that balances the joinery, or behind the front desk where it is out of sight - both of which are wrong for reasons of air rather than taste. Put the requirement in the brief in the designer's own language: near a supply grille, at least a couple of metres from any return, angled into open volume, and not at a workstation. That gives them a zone to design within rather than a fight at the end. It also lets them do the interesting part properly - a recess, a slatted panel, a console detail - because they know where the object has to live before they draw the elevation.
Tip: Send them the grille layout with the reflected ceiling plan. Most schemes are drawn without anyone looking at where air enters.

Put the unscented rooms in writing
This is the item with consequences, and it is always the one omitted
Give the designer a written list: procedure and graft rooms, post-procedure and recovery areas, and photography or documentation rooms are unscented - not lightly scented, and no provision made for any future diffuser. Without that list somebody will thoughtfully detail a niche for a diffuser in recovery, or specify a shared duct run that reaches a graft room, and it will be built before it is questioned. Post-operative scalp is an open wound and a photography room is producing a clinical record; these are environmental precautions, not stylistic choices. Add one line that the treating clinician's judgement overrides the drawing in their own rooms, and the whole issue is settled before site.
