Scent is the one part of a clinic environment that has three natural owners and usually ends up with none. It arrives after handover, as a box on the reception desk, next to the sanitiser bottle and the card machine.

Founders ask me this expecting me to say the branding agency, because scent sounds like a branding subject. It is mostly not. The two decisions that actually determine whether a clinic smells the way you intended are made by the services consultant and the interior designer, months before anyone writes a word about brand. The agency's contribution matters, but it comes last and it is a writing job rather than a design one.
The pattern I see over and over is that none of the three is asked. Everyone assumes scent is a small late purchase, which it is, and therefore that it has no design consequences, which is wrong. By the time somebody orders a machine, the ceiling is closed, the joinery is fitted, the only free socket is behind the desk, and the unit ends up in the one position in the room where it can do least. Nothing is ruined by that - a retrofit is perfectly workable and most clinics I advise are retrofits - but a ten-minute conversation at services stage would have bought a much better position for nothing.
