The first test: hold a strip of tissue against the fan grille with the door shut, then with it open fifty millimetres. If it holds with the door open and falls with it closed, you have found your problem in ten seconds.
The habit that matters most: the fan should run for twenty minutes after the shower, not during it.
2. Then: the fan is undersized for its duct run, or it is not discharging outside at all. A small axial fan rated for a short straight duct will move a fraction of its rated air through four metres of flexible ducting with two bends and a crushed section above a false ceiling. Worse, a surprising number of bathroom fans in flats discharge into the false ceiling void or into a shared vertical shaft with no back-draft flap — in which case the moist air either sits in the void, or goes into the shaft and comes back to you, sometimes with a neighbour's air behind it. Confirm it by following the duct. Open the false ceiling access panel if there is one and look for a duct that actually terminates at an external louvre; look for damp staining or drips on the ceiling boards near the fan; and hold a tissue at the outside louvre or the shaft opening while the fan runs to check that anything is coming out. If air is not leaving the building, no amount of run-on time will help.
3. Then: permanently wet grout and silicone. The surfaces are clean; the joints are not dry. This is the cause that survives daily cleaning, because cleaning a bathroom means cleaning its faces and the smell lives in its joints. Grout is porous and stays damp for hours after the tile beside it is dry. Silicone beads — where the wall meets the floor, around the shower tray, along the counter and behind the WC pedestal — hold water in the micro-gap behind them. Once a silicone bead has black spotting in it, it is colonised right through and no cleaner will fix it; you are wiping the top of something that is growing inside. Confirm it with the dry-room test: squeegee and towel the whole bathroom completely dry, leave it six hours with the fan running and the door open, and then get down and smell the grout line at the base of the shower wall, the silicone at the floor junction, and the joint behind the WC. If the room is dry and those lines still smell, that is your source.
4. Then: a floor trap that is not the one you clean. Almost every bathroom has more drains than the owner thinks. There is the shower trap you clean daily, and then very often a separate floor trap near the washbasin or the WC, sometimes one behind the WC under a grating, sometimes one under the vanity that was tiled around and is now half hidden, and sometimes a standpipe for a washing machine. The one you clean is fine. The one nobody uses has an evaporated water seal, and a dry trap is an open pipe to the building's drainage. Confirm it by pouring a mug of water down every grating you can find — get on your knees with a torch and look behind the door, under the vanity and behind the WC. Note which ones gurgle or refuse to take water. If the smell drops noticeably for two or three days afterwards, you have found a dry trap, and the fix is to keep water in it.
5. And the two things here that are not fragrance problems at all. Black spotting on silicone, grout or the ceiling is mould. That is a health issue and a moisture problem, not an odour — the answer is to get the room drying properly and to cut out and replace the colonised silicone, not to scent over it. And a distinctly sewer-like or eggy smell, particularly one that is worse late at night when the building's stacks are quiet, is a plumbing fault: a dry trap, a failed seal at the WC pan connector, or a blocked soil vent. That is a plumber, not a diffuser. Musty and sewer-like are different smells and it is worth being honest with yourself about which one you have, because they lead to completely different work.

Why daily cleaning cannot reach any of this
A bathroom is the only room in a home whose surfaces are deliberately soaked every day, and the only room where the thing you are fighting is not soil but water. Cleaning removes soil. It does not remove water — in fact a wet-mopped bathroom with a closed door and a fan that stopped ten minutes ago is wetter after cleaning than before. Everything on the shortlist above is a water problem wearing a smell as a symptom. The grout is damp, the silicone has water behind it, the trap has lost its water, the fan is not carrying water away, or the duct is delivering water into a ceiling void. That is why a daily clean can be genuinely thorough and completely ineffective at the same time, and why the reader who has already tried harder cleaning is right to feel that something else is going on.
The other half of it is that a windowless bathroom has no fallback. In every other room in the flat you can open something, and five to ten minutes of cross-ventilation will displace more air than the previous several hours. A bathroom with no window has exactly one route out for its air, and that route only exists while a small motor is running and only works if air can get in behind it. An extract fan is a pump with two ends, and almost all attention goes to the end you can see. The inlet — the gap under the door, or the louvre in it — is invisible, unglamorous and usually missing, because doors are ordered to look neat and a fifteen-millimetre undercut does not look neat. When the room is sealed, the fan does not stop working in a way you would notice. It quietly moves a small fraction of what it should, at the same volume and with the same vibration, for years.

The plan for this bathroom
Week one: fix the air path, because nothing else works until air can move. Do the tissue test today, both ways. If the door is sealed, get a fifteen to twenty millimetre undercut planed off it or a louvre fitted — an hour of joinery, and it is the single most useful change on this page. Until that happens, prop the door open fifty millimetres and run the fan for twenty minutes after every shower. Fit a run-on timer to the fan if you can, or give it a separate switch so it is not tied to the light. Then follow the duct with a torch: check that it terminates outside the building, that it is not crushed or full of bends above a false ceiling, and that it has a back-draft shutter if it enters a shared shaft. Add a squeegee to the shower and use it on the walls and floor after every use — sixty seconds, and it removes most of the water the fan would otherwise have to carry.
Week two: deal with the joints and every drain. Run the dry-room test to confirm the grout and silicone, then cut out and replace any bead with black spotting in it, using sanitary-grade silicone on a completely dry gap. Seal the grout if it is old and porous. Find every drain in the room with a torch, pour a mug of water down each, and put a weekly reminder on your phone for the ones you never use — a dry trap is an open pipe, and refilling it costs ten seconds. Get the towels out of the room to dry, or at least onto a rail with air on both sides rather than folded double on a hook. Empty and wash the bin including the lid and its underside, and dry it. Keep bottles off the floor of the shower, because the ring of water under a shampoo bottle never evaporates.
Then the fragrance, and this is the one room where I will argue for it rather than talk you out of it. Every other room in a flat has a ventilation moment — a window you open, a draught that crosses it, a morning purge. A windowless bathroom has none, which means it has no natural way to acquire a pleasant smell either; the best it can do on its own is smell of nothing. I get this question more than almost any other, and it usually arrives with an apology — people assume they must be missing something obvious about cleaning. They are not.
