Why does my fully air-conditioned chiropractic clinic sometimes smell closed?
Because your air conditioning recirculates rather than ventilates, and those are two different pieces of engineering. A split unit, a cassette or a high-wall - the systems in almost every Indian clinic I get sent photographs of - draw air out of the room, pass it over a cold coil, and push the same air back into the same room. Heat is removed. Air is not exchanged. Nothing arrives from outside and nothing leaves, which means everything the building releases and everything its occupants bring in stays in circulation and climbs steadily through the day. That is the physical basis of the word "closed", and it is the reason a beautifully cooled clinic at four in the afternoon can read more shut in than an un-cooled room with a window open. The give-away is that the sensation lifts the instant somebody props the door, then returns within ten minutes of it closing. Turning the fan up does not help. Faster recirculation moves the same air more quickly past your nose; the total is unchanged. Lowering the set point does not help either. The only fix is an exchange, and there are two honest routes to one. If anything in the building opens, build a fresh-air routine into the day: two deliberate breaks, one late morning and one mid-afternoon, with openings at both ends and internal doors propped so the building has a path. The mechanism
A cold coil is not an opening to the outside
A split unit has an indoor head with a fan, a coil and a filter, and an outdoor unit that dumps heat. The refrigerant loop crosses the wall. The air does not. Room air is pulled in at the top of the indoor head, chilled, and blown back out at the bottom, over and over, all day. The filter catches dust and lint and needs cleaning, and cleaning it is worth doing for the sake of the machine, but a lint filter is not an exchange either. So the honest description of a fully air-conditioned clinic is a closed volume being held at a comfortable temperature. Everything else about the air is whatever you and your building put into it.
Tip: The quickest test is to prop the front door for two minutes and stand still. If the room changes immediately and reverts ten minutes after you close it, you have a recirculation question rather than an odour one.

The routine
Two deliberate breaks, ten minutes each, doors propped
Put them where the schedule allows: late morning between clients, and mid-afternoon. Openings at both ends of the building, internal doors held open so there is a path rather than a set of pockets, and the air conditioning off or on high fan while it happens. Ten minutes twice a day changes a sealed room noticeably, and it costs nothing except a warmer reception for a quarter of an hour. If the building has no operable window, this becomes a question for whoever maintains the system: is there a fresh-air intake, is it open, and what proportion of supply is outside air. That is a facilities engineer's answer, not mine, and I would rather point at them than guess.
Tip: Put the breaks in the appointment diary as blocks. A routine that depends on somebody remembering it lasts about a fortnight.

The format
In a closed loop, water has nowhere to go either
This is the one place a product choice genuinely follows from the physics. An ultrasonic diffuser works by aerosolising a water-based fragrance, so it puts a small amount of water into the room along with the scent. In a room with an air exchange that is unremarkable. In a sealed recirculating room it stays in the loop, and over a long run in a humid city it can show up as a film on glass, on chrome and on vinyl. A waterless nebuliser adds no water, needs no tank cleaning cycle and leaves nothing behind. Under about 750 sq ft the same is true of a reed diffuser, which adds no water either and costs a fraction of the price. Neither of them does anything to the air itself, and neither is offered as though it does.

Frequently asked questions
Does my split air conditioner bring in any outside air at all?
Almost certainly not. A standard split, high-wall or cassette unit has no duct to the outside for air - only refrigerant pipes and a drain. The exchange you are looking for has to come from an opening or from a ducted system with a fresh-air intake. Whoever installed the equipment can tell you in one sentence which of those you have.
Is cleaning the air conditioner filter worth doing for the smell?
Worth doing for the equipment, and worth doing because a loaded filter that has been damp is itself a candidate source. But a lint filter does not exchange air, so a clean filter will not change a closed-feeling room on its own. Treat it as maintenance rather than as the answer to this question.
Should I run the fan on high all day to keep the room moving?
It moves the same air faster past you. That can feel better and it does not change what is in the room, because a recirculating unit has nothing to replace it with. Save the high fan setting for the ten minutes when the doors are propped, which is when moving air actually achieves an exchange.
Is an ultrasonic diffuser a bad idea in a fully sealed clinic?
It is the less suitable of the two formats there, for one reason: it aerosolises water into a room with no route out, and over time that can show as a film on glass and chrome and mean a regular tank routine. A waterless machine or a reed adds none. That is a maintenance judgement, not a statement about anyone's air.
