Founders usually think of this as scented rooms and unscented rooms. There is a third tier in between, it is where most of the mistakes happen, and the thing that decides which tier a room belongs in has nothing to do with how much you spent on it.

Measure the dwell time honestly
Ambient scent is designed for ninety seconds, not forty minutes
Write the real number next to each room on your plan - not the number in the brochure, the number a patient actually experiences. Reception: a minute or two. Waiting: fifteen to forty minutes, sometimes longer on a busy morning. Consultation: thirty to sixty. Procedure: most of a working day. The whole design intent of ambient fragrance is the short encounter, where somebody registers a clean room on arrival and has stopped thinking about it by the time they have sat down. The longer the dwell time, the less any fragrance is doing for the person in the room and the more it is simply present. That is why tier one can carry a level that tier two should not, and why tier three carries none.
Tip: Count the busiest morning, not the average one - that is the dwell time your plan actually has to survive.

The less choice they have, the less you are entitled to add
This is the variable clinics almost never write down, and I think it is the more important of the two. A person in your reception can step outside for air without explaining themselves to anybody. A person in a consultation can bring it to a close. A person in a procedure chair, halfway through extraction, cannot - and neither can a patient in recovery with a dressed scalp. The scenting decision should get more conservative exactly as the patient's ability to opt out disappears, because that is the point at which their preferences stop being expressible. It also explains the odd case where a short-dwell room still belongs in tier three: a photography room takes ten minutes, but a patient being documented has very little practical choice about being in it.
Tip: If a patient would need to interrupt a clinician to leave the room, treat it as tier three.

Real clinics have rooms that will not sit neatly in one tier. A consultation cabin that is a conversation at eleven and a donor-area examination at three. Almost every founder who asks me this has already sorted their clinic into two piles - the rooms patients see and the rooms they are treated in - and expects me to confirm the split. The two-pile version is where the mistakes live, because it forces the consultation lounge into one pile or the other and neither answer is right. There is a middle tier, it gets drift and nothing else, and once you name it the rest of the plan stops being contentious.
What surprises people is what decides the tier. Not the finish, not the budget, not how many patients pass through. Just how long somebody sits there and how easily they could get up and leave. Those two happen to move together in a clinic - the longer you are in a room, the less able you are to walk out of it - which is why the tiers come out so cleanly.
