Air change: how many times an hour the air in the room is genuinely replaced, which in most Indian clinic fit-outs means whether there is a working extract fan, whether an openable window is ever opened, and whether the split unit is recirculating the same air all afternoon. Laundry and equipment: whether used towels, mats, belts and bolsters leave the room after each session or stack in a corner until the end of the day. Fix those three and the room smells of nothing much. Leave them and no fragrance rescues it, because - and I want to be exact rather than polite here - fragrance never covers, neutralises or hides another smell. It is an added smell, not a subtractive one. The scented zone is the entrance, reception, waiting and whatever the corridor takes from it. One profile, clean and dry, at a level low enough that a patient registers a clean room rather than a fragrance. Judge it by walking in from outside after a break, and again from the corridor with a cabin door open, because that diluted version is what most of your patients spend real time with. Put the machine at the far end of reception from the corridor mouth, run it on clinic hours only, and engage the key-lock so the setting you chose in January is still the setting in July. And keep one group of chairs effectively unscented by design.
Tip: If a patient can name the fragrance, it is already too strong.

Cabins - a rule, not a decision
Nothing at all, and no lighter version of the rule
A physiotherapy cabin is an examination room for scenting purposes, whatever the door says. Close manual work, partial undress, forty-five minutes to an hour, and a door the patient cannot simply walk out of. No reed, no plug-in, no small diffuser on the windowsill. A faint drift from reception through an open door is unavoidable and acceptable; a source inside the room is not. The changing area follows the same rule for the same reason. What both rooms actually need is ventilation and a proper cleaning routine, and no fragrance is ever a stand-in for either of those.

A room with six people working in it, a split unit recirculating the same air and a pile of used mats in the corner smells the way it does for mechanical reasons. Nobody writes to me about the reception in a clinic like this, and nobody writes about the cabins. They write about the exercise room, usually in the same week they have been quoted for a machine to put in it. And the thing I have to say is not the thing they want to hear: a diffuser in that room will not do what you are hoping, because fragrance is additive. It does not take anything away. You would end up with what was there plus a fragrance, and the combination is worse than either.
What I ask instead is three dull questions. How many people are in that room at once. How many times an hour is the air genuinely replaced - is there a working extract, does anyone open the window, or is a split unit moving the same air around all afternoon. And where do the towels, mats and bolsters go after a session. In nearly every case the answer to one of those three is the whole explanation, and none of them is a fragrance question. Those are your facilities engineer's decisions rather than mine.
