Wet floor mats and the carpet underneath
This is the No.1 culprit. Rubber or fabric mats trap the water your shoes bring in, and the water soaks through into the carpet underlay - a spongy layer that stays wet for days because no air reaches it. You dry the visible carpet, the smell fades, and then it returns, because the underlay is still damp. Lift the mats out, sun-dry them, and press a dry towel hard into the carpet to pull moisture from underneath.

A parked car in monsoon is a sealed box of humid air. Fabric seats and the roof lining hold that moisture, and with the windows up all day it never escapes. This is a low, general mustiness rather than a sharp smell from one spot. Air the car out - park with windows cracked when it is safe and dry, and run the blower on fresh air to exchange the stale, humid cabin air. The boot and spare-wheel well
Water pools in the spare-wheel well under the boot floor and sits there unseen for weeks. Wet reusable bags, a damp umbrella or gym kit left in the boot add to it. Because you rarely look there, the mildew grows undisturbed. Lift the boot floor, check the spare-wheel well for standing water, dry it, and never store damp items in a closed boot.
