Five, and I would write them on the scenting plan rather than trusting anyone to remember them. One: procedure and graft rooms. Extraction, implantation, graft handling and PRP - anywhere the procedure itself happens. The patient is there for hours, the scalp is open, and your team breathes that air every working day. Two: post-procedure and recovery. This is the longest dwell time in the building and the patient cannot simply get up and walk out of it, which makes it the strongest entry on the list rather than an afterthought. Three: photography and documentation. It is a clinical record room; nothing is gained by scenting it and a patient sitting for before-and-after images is already being asked to feel exposed. Four: any consultation room where a physical examination happens. This is the entry founders push back on, and my answer is that it depends on what the room actually does. A consultation lounge that is a conversation and a screen may carry a faint drift from the corridor. A room where you part someone's hair, run a densitometer over the donor area or take close-up images is doing clinical work, and it belongs on the list. Five: at least one seating area in the waiting room, kept effectively unscented by choice. That one is a policy rather than a consequence, and it exists so that a patient who does not want to sit in a scented room can simply sit somewhere else without having to raise it with your front desk. Two things the list is not. How to write the list so it survives
A list that lives in the founder's head lasts about a fortnight. Three things make it survive contact with a real clinic and a rotating staff.

Put it on paper and give it to facilities
A rule nobody can read is a rule nobody follows
Write the five areas out, print the list, and hand it to whoever manages the building - housekeeping supervisor, facilities manager, clinic administrator, whoever actually opens and closes. It should say the areas by their names on your floor plan rather than in general terms, and it should say no diffuser, no reed, no spray, no plug-in and no scented cleaning agent chosen for its smell in those rooms. The reason this matters is turnover. The person who understands your reasoning today is not necessarily the person restocking the store cupboard in eight months, and a printed list survives a handover in a way a conversation does not.
Tip: Put the list on the same sheet as the machine's settings, so whoever changes one sees the other.

Decide the consultation rooms case by case
The test is whether an examination happens in there
This is the only entry that needs judgement rather than a rule, and the test I use is simple: does clinical work happen in that room? A consultation lounge where you sit across a desk, talk through a plan and look at a screen is a conversation room, and a faint drift from the corridor is acceptable there. A room where you part the hair, examine the donor area, use a densitometer or take close-up images is doing clinical work, and it goes on the fragrance-free list regardless of what it is called on the drawing. Where a clinic uses one room for both, treat it as clinical - the stricter reading costs you nothing, and the treating clinician's judgement in their own room outranks this or any other plan.
Tip: If the same room is a consultation at eleven and an examination at three, it belongs on the list.

Make the unscented seat a standing policy, not a favour
Somebody should be able to avoid the fragrance without asking
The fifth entry is the one that separates a clinic that has thought about this from a clinic that has not. Keep at least one seating bay in the waiting area effectively unscented - furthest from the machine, out of the air path, ideally near a door or a window - and treat it as a standing arrangement rather than something the front desk offers when a patient complains. Fragrance sensitivity is real and is never minimised, and no product should ever be described as hypoallergenic or safe for sensitive individuals, ours included. The practical value of the unscented bay is that it removes the need for anyone to have a conversation about it at all, which is worth more than any wording your staff could be trained to use.

What founders usually send me is a floor plan with the rooms they want scented already highlighted, and a question about which fragrance suits them. I almost always send it back with the opposite thing marked. The exclusions are the part that has to be right, because a fragrance in the wrong room is not fixed by choosing a better fragrance, and the rooms that need excluding are not obvious until somebody says them out loud.
The list itself is short and I have never had to change it. Procedure and graft rooms. Post-procedure and recovery. Photography and documentation. Any consultation room where an examination actually happens. And one seating bay in the waiting area, kept unscented on purpose so that nobody has to ask. The first four come from what those rooms do. The fifth comes from the fact that fragrance sensitivity is real, that nothing should ever be sold as hypoallergenic or safe for sensitive individuals, and that a patient should not have to make a request in front of a full waiting room to avoid something they did not choose.
The pleasant surprise, most of the time, is the bill.
