Projection is one of those words that gets used as though it were a personality trait a bottle either has or lacks. It is not. It is the measurable distance at which a fragrance is detectable, it is produced by a short list of physical factors, and you control nearly all of them. Which means the question is less about what to avoid buying than about what to stop doing.
Quick answers — read this first
Should a dentist avoid high-projection fragrance during patient hours? Yes. And more directly: during patient hours the answer is little or nothing at any projection level, because the person in the chair cannot move away from it. Your practice's own fragrance policy decides before any of this — the institution rule is argued in full here.
What actually makes a fragrance project? Six things, in rough order of importance: whether it has an alcohol carrier, how much you applied, how high on the body you applied it, how warm that skin is, whether it went onto fabric, and only then the composition itself. Five of those six are set by you rather than by the bottle.
Does an oil attar project less than a spray? Yes, and for a structural reason rather than a stylistic one. Alcohol is a carrier that lifts fragrance off skin and into the air; an oil has none, so it warms on the skin and stays close. SOSA's own pages call these skin scents rather than room scents. Short answer: Yes, avoid high projection during patient hours — and then go further, because at chairside distance the honest answer is little or nothing whatever the projection level. The practice's policy decides that, not us.
The mechanism: Projection is alcohol, quantity, height, skin warmth and fabric before it is composition. An oil attar removes the first of those entirely, and a single dab at a low, covered placement removes most of the rest.

Should dentists avoid high-projection fragrances during patient hours?
Yes — and the honest version of the answer goes further than the question does. During patient hours the recommendation is little or nothing at any projection level, because projection is only half the problem. The other half is that a reclined patient cannot move away, did not choose what you are wearing, and is unlikely to mention it either way. Your practice's or employer's own fragrance policy decides this before we do, and where a practice is fragrance-free that is the end of the matter.
But the word deserves a proper definition, because most people use it to mean 'strong' and that is not what it means. Projection is the distance at which a fragrance is detectable by someone who is not wearing it. It is different from intensity, which is how concentrated it smells at the skin, and different again from longevity, which is how long it lasts. A fragrance can be intense on the wrist and project almost nowhere; that combination is precisely what an oil attar is. And a fragrance can be modest on the skin and fill a corridor, which is what a lightly-worn alcohol spray often does.
Six things drive projection, and five of them are decisions you make rather than properties you buy. First and largest is the carrier. Alcohol is volatile: it evaporates fast and takes fragrance molecules upward and outward with it, which is the entire reason sprays announce someone before they arrive. Second is quantity — two dabs is not a small increment. Third is height: the same dose at the base of the throat travels further than at the inner wrist because it is uncovered, warm, and pointed at the face of whoever is opposite. Fourth is skin temperature, which is why the same application reads louder in a warm surgery in May than in an air-conditioned room in December. Fifth is fabric, which holds fragrance far longer than skin and re-releases it as you move. Only sixth, and last, is the composition itself.
Which is why the format answer comes before the bottle answer. An oil attar removes the biggest driver on the list by not having it: there is no alcohol to carry anything anywhere, so the oil warms on the skin and stays close. And the one bottle this page names in order to rule out.Nawaab is a high-projection fragrance and SOSA says so on its own product page: not a daytime office attar, but a presence scent. That is an honest description rather than a warning — there are evenings and weddings where exactly that is wanted. It has no place in a dental working day, and neither does anything else built to carry. If you want a way to check where a fragrance you already own actually sits, the arm's-length test takes ninety seconds. Nobody can promise that any of this changes how you are perceived or treated by a single patient, and this page does not try to.

The carrier
Alcohol is a delivery system, and you can decline it
An alcohol spray is engineered to evaporate. As it goes it lifts fragrance molecules off the skin and disperses them into the room, which is why a sprayed scent arrives before its wearer and lingers after them. An oil attar has no such mechanism. It warms with the skin and stays on it, which is why the same nominal strength reads completely differently in the two formats. Nothing else on this list moves projection as far as changing the carrier does.
Tip: If you already own a spray you like, wearing it in the evening rather than the surgery is the simplest possible fix.
Adaa is alcohol-free and DPG-free — ₹379 for 3ml, the lightest of the four attars.
Two dabs is not a ten per cent increase at conversational distance; it is a different fragrance. And height changes the result as much as quantity does. The base of the throat and behind the ears are uncovered, warm and aimed forward at the face of whoever is opposite you. The inner wrist and the inside of the elbow are low, partly covered by a sleeve and effectively aimed at the floor. Rubbing wrists together after application spreads the oil over more warm skin and raises projection for no benefit at all.
Tip: Move the application down and inward before you reduce the dose any further. It is usually enough on its own.

Two amplifiers that have nothing to do with the bottle
Warm skin raises projection — the same application reads further in a warm surgery in the middle of a long list than it does in a cool consult room first thing. That is also why the published wear figure is 6–8 hours on skin, longer on warm or oily skin: heat changes the curve. Fabric is the other amplifier, and the more significant one for anyone in a tunic. Clothing holds fragrance far longer than skin and releases it as you move, so a garment is dosed for days rather than for a shift.
Tip: Skin only. A tunic that has been dosed is carrying every application made in it since it was last washed.
Beast at ₹549 is the other well-stocked tin — an evening register, deliberately fuller than Adaa.
Projection is not a property a fragrance has. It is something a wearer does with a fragrance — with a carrier, a quantity, a position, a temperature and a garment. Five of those are yours.
Which is why swapping bottles is the last thing to try, and almost always the most expensive way to solve a problem that one dab and a different wrist would have fixed.

Projection is the single most misunderstood word in retail perfumery, and the misunderstanding is profitable, so it does not get corrected often. People are sold a 'low-projection' bottle and then apply it twice at the throat on a hot afternoon, which undoes the purchase entirely. The bottle was never doing most of the work.
What genuinely changes projection is the carrier. An oil has no alcohol in it, so there is nothing to lift the fragrance off your skin and distribute it around a room. That is why I make attars rather than sprays, and it is why the format suits people who work at close range for a living. It is physics rather than philosophy.
