Musty is not a smell you can treat as a smell. It is a report that something in your building is wet and has been wet for a while, and every rupee spent on fragrance in that situation is spent on the wrong end of the problem.

Almost every musty clinic I am written to about turns out to have one of these three, and the third is the one people find last because it is behind a door that is always shut.

Start with the fabric and the floor at the entrance
A monsoon entrance takes more water than anything else in the building
Every patient who walks in during July brings water with them, and the first three metres of your clinic absorb it - the mat, the carpet or its underlay, the edges of any rug, and the legs and lower upholstery of whatever seating sits nearest the door. That material then spends the day in an air-conditioned room at high ambient humidity and never quite dries, which is exactly the condition that produces a musty report. The remedy is unglamorous: a larger absorbent mat changed and dried daily rather than left in place, hard flooring rather than carpet in the first stretch if you are ever refitting, upholstery pulled away from external walls so air can move behind it, and where possible a dehumidifier running in that zone. Fabric near a monsoon entrance is a reservoir that recharges every single day, which is why it outlasts every other intervention you try.

Check every space that has no air moving through it
Still air plus humidity is the whole recipe
Musty concentrates wherever air does not move, and a clinic has more of those spaces than a home: the corridor cupboard, the store room, the space under the reception counter, the cabinet under a wash point, the linen or towel store, the gap behind a bank of seating. Open every one of them on a wet afternoon and put your hand inside. If the air in there is noticeably heavier than the room, that is a source and it has been quietly feeding the whole floor. Fixes are simple and cheap - leave doors ajar, take things off the floor of the cupboard and onto shelves so air can pass underneath, remove cardboard entirely because cardboard holds water beautifully, and put a small vent or louvre in a door that has none. The store room is the single most common finding, and it usually contains cardboard, textiles and something that was put away slightly damp.
Tip: Cardboard is the material to remove first. It absorbs, holds and reports moisture better than almost anything else in your building.

Get anything structural looked at rather than managed
A leak is not a smell to live with until October
If the smell is strongest near one wall, if it worsens specifically when it is raining rather than merely when it is humid, if there is any staining, blistering paint or a cold patch on a skirting, or if it lives under a sink or behind a wash point, then you are not looking at a seasonal nuisance - you are looking at water entering or escaping somewhere, and that belongs to a building contractor now rather than in September. I am not qualified to advise on any of it and I will not pretend otherwise; in a building that does clinical work there may also be requirements from your surgeon or facilities engineer about how affected areas are handled, and those come first. What I can tell you is that no fragrance product on the market changes anything about a wet wall, and that anyone who tells you theirs does is selling you something they should not be.

Monsoon is the season when I get the most enquiries and refuse the most of them. The email is usually urgent - the clinic smells musty, patients are arriving, what is the strongest thing you sell - and my answer is that musty is not a smell, it is a moisture reading, and there is nothing in my catalogue that removes water from a building. I would rather say that in July and keep the relationship than sell somebody a machine in a wet month and have them conclude, correctly, that it did nothing.

What I do ask people to do is walk the clinic on a wet afternoon with their hand out. Touch the carpet inside the door, the back of the sofa, the inside of the corridor cupboard, the cabinet under the wash point. Something will be cold and slightly damp and it will be the answer. Then it is mats changed and dried daily, cardboard out of the store room, cupboard doors left ajar, a dehumidifier in the affected zone if your building allows one, and a contractor for anything involving a wall or a leak. None of that is my field, and I am not going to pretend it is.

The one thing I can tell you that is genuinely useful is a constraint rather than a recommendation. In a humid month, do not run a water-based ultrasonic - it is adding moisture by design, in a room you are trying to dry.
