This is the post the other nine in this family point at. It is a gate rather than a guide, and the honest outcome for a good number of the clinics that read it is that they walk away from the purchase and fix a drain instead.

When is a clinic ready for ambient fragrance? When all seven of these are true: nothing in the building is damp; the air genuinely changes at least twice a day and you know whether your AC brings any in; cleaning is done at the specified dilution and outside patient hours, with the cupboard closed and off the patient floor; there are no open reservoirs - bins, stock, drains, traps; soft furnishings are on a written cleaning schedule; every existing fragrance source has been removed for a fortnight; and a cold-nose walkthrough on several mornings found no source. All seven. Not most of them.

What happens if one of them fails? You spend the money on that instead, and you come back to the scenting question afterwards - if you still want to. That is not a soft recommendation. A clinic that scents over a failed condition ends up with the original problem plus a fragrance, which patients read as worse than the problem alone, and which your staff sit in all day. Fragrance is never a mask and never a substitute for cleaning, ventilation, repair or infection control.

Straight answer

Before every single time, and the way to make that operational rather than pious is to write it as a gate with seven conditions that all have to be true. One: nothing in the building is damp. No musty smell anywhere, no leak, no cold wet patch on a skirting, no fabric that has never dried since the last monsoon. Two: the air genuinely changes. The clinic is opened and aired at the start of the day and once more at the quietest point, you know whether your AC has any fresh-air intake at all, and the filters have a service history and a schedule - which are your HVAC contractor's and your surgeon's decisions, not mine. Three: cleaning is right. The manufacturer's specified dilution, confirmed with your infection-control lead and measured rather than estimated; heavy cleaning outside patient hours; twenty minutes of moving air immediately afterwards; the cupboard closed and, ideally, off the patient-facing floor. Four: there are no open reservoirs. Lidded bins emptied at the lunch gap as well as at close, stock and consumables in closed storage, no decanted bottle with a loose trigger, and every drain and trap in the building wet and sealed. Five: the soft furnishings are on a written cleaning schedule - seating, curtains, blinds, carpet - because fabric holds and re-releases for weeks and will hold whatever you add next just as faithfully. Six: there are no competing fragrance sources. Everything plugged in, stuck up or hidden in a drawer has been removed and the building has run on nothing for a fortnight, so you know what it actually smells like. Seven: a cold-nose walkthrough on three or four separate mornings found no source. The three failures that stop a purchase

Of the seven conditions, three account for nearly every clinic that should not buy this month. If you only audit three things, audit these.

Anything damp is an automatic fail
Moisture is a building problem and fragrance has no relationship to it
If there is a musty smell anywhere in the clinic, a leak under a wash point, a cold damp patch on a skirting, blistering paint, cardboard that has taken water in a store room, or upholstery that has not properly dried since the last wet season, then the gate is closed and nothing else on this page applies. Damp belongs to a building contractor and, where it touches clinical areas, to your surgeon and facilities engineer - not to me and not to any fragrance supplier. The reason this is an automatic fail rather than a caution is that damp is progressive: it does not stay the size it is now, and a scent machine bought this month simply becomes a machine running in a wetter building next month. Fix the water, dry the materials, then reopen the question. If you were going to spend ₹11,999 with me, a dehumidifier and a plumber is a better use of it.
Tip: A smell that changes when it rains, rather than when it is merely humid, is a leak until a contractor tells you otherwise.

Air that does not change is an automatic fail
Cooling is not ventilation, and a sealed building cannot be scented honestly
Second automatic fail: a clinic whose air is not genuinely replaced. Split and cassette units recirculate the same air at a different temperature and introduce no outside air at all, so a fully air-conditioned clinic can be entirely unventilated - and adding fragrance to unrefreshed air gives you unrefreshed air with a fragrance in it, which your staff breathe for nine hours while patients pass through for forty minutes. The tick you are looking for is concrete: the building is opened and aired at the start of the day and once at the quietest point; you have asked your HVAC contractor in writing whether there is any fresh-air intake and whether it is open; and the filters have a service history and a schedule. What those numbers should be is their engineering judgement and your surgeon's requirement, never a fragrance supplier's, and I will not offer an opinion on either.
Tip: If nobody in the building can answer the fresh-air question, that is a fail until somebody can.

An unidentified smell is an automatic fail
You may not scent over something you have not looked for
The third and most common fail is the simplest: there is a smell, and nobody has done the work of finding out what it is. The remedy is free and takes three mornings - be the first person into the building with a cold nose, doors as they were overnight, one room at a time in a fixed order, low at floor level and high at the AC return, every cupboard opened, every bin, drain and trap checked, one written line per room, repeated on several different days. Most clinics find something ordinary: a dried-out trap, the bin under the counter, a store room with no air movement, laundry waiting for collection. All cheap to fix. Until that walk has been done properly, any purchase is a purchase made in the dark, and I would rather you kept the money and spent an hour on three consecutive mornings.

I wrote this gate because I kept giving the same answer in different words, and because a supplier who never says no is not giving anybody information. Nine posts in this part of the cluster describe individual problems - stale mornings, disinfectant, monsoon, upholstery, four fresheners competing in one reception - and they all converge here. Seven conditions, all of which must be true. Not most of them, not five of seven with a note about the other two. A gate that opens for six out of seven is not a gate.

The three that fail most often are damp, air that never changes, and a smell nobody has actually looked for. Each of those has a proper owner who is not me: a building contractor, an HVAC contractor working to your surgeon's requirements, and an hour of your own time on three consecutive mornings with a notebook. When a clinic fails one of them, my recommendation is the failed line itself - a plumber, a dehumidifier, a filter service, an upholstery cleaner, four lidded bins - and I would rather send that email than an invoice, because a machine installed over a problem produces a clinic with the problem and a fragrance in it, and everyone who works there sits in the result all day.
