Three of these four options end with you buying something from me. The fourth does not, and it is the one I think most clinics under-consider, so I have given it the same space as the others rather than the polite paragraph it usually gets.
Quick answers — read this first
Which of the four is right for a hair clinic? Fresh and dry-woody are both correct and largely interchangeable. Hotel-like is correct only in half of its range - the cool half. Completely neutral is also correct, and it is the option this industry never puts on the list, because there is nothing to sell in it. My honest ranking for a surgical hair clinic is: dry-woody or fresh first, depending on your interior; completely neutral a close and entirely respectable second; and hotel-like only if you are disciplined about which half of it you take.
What is wrong with hotel-like? Half of it. Hotel scenting has a cool register - white tea, green notes, dry citrus, light woods - which is exactly right for a clinic, and a warm register - amber, resin, soft musk, sweet woods - which is exactly wrong. Most people asking for a hotel smell have the warm half in mind, because that is the half that reads as expensive in a lobby at nine in the evening. In a building where grafts are extracted, warmth reads as a bedroom or a spa. Take the cool half and hotel-like becomes a perfectly good answer.
What does each option cost? Should a hair clinic smell fresh, woody, hotel-like or completely neutral?
All four are defensible, which is not the answer people expect, so let me take them one at a time and then give you a verdict. Fresh works. A dry light citrus, white tea, a green note - clean, cool, unfussy, and close to impossible to object to. It is the safest of the four and the easiest to get wrong only by overdoing it. Dry-woody works. Cedar, pine, sage, something austere and slightly resinous but never warm. It sits better against a dark or timber-heavy interior, and between the two there is little to choose, so pick by palette. When a founder tells me they want their clinic to smell like a five-star hotel, my first question is which hotel and at what time of day. The bright cool lobby register - white tea, green notes, dry citrus, light woods - transfers to a clinic almost unchanged. The warm evening register - amber, resin, soft musk, sweet woods - does not, because warmth signals hospitality and comfort and a clinic is signalling competence and cleanliness. The warm half is also the half that reads as expensive, which is precisely why people ask for it. Ask for the cool half by name and hotel-like becomes a good answer instead of a risky one.
Tip: If the description of a fragrance would suit a hotel bar, it will not suit your reception. If it would suit the same hotel's breakfast room, it probably will.

Take neutral seriously, and then test whether you can actually hold it
Most clinics that think they are neutral are running four fragrances at once
Completely neutral is a legitimate answer and I would not want anybody to reach it by default. Do it deliberately: unscented or near-unscented cleaning products, no room freshener anywhere in the building, no plug-in in the toilets, unscented hand wash on the counter, and enough ventilation that the room does not develop a character of its own by the afternoon. That is a real operational discipline, and it is more work than running one machine. What is not neutral is a clinic with no scenting policy at all, where the floor cleaner, the toilet freshener, the hand wash and somebody's desk candle are negotiating with each other every afternoon. That is not restraint, it is four fragrances with no plan.
Tip: Walk the whole clinic at four in the afternoon and write down every smell you can name. If the list has more than one item on it, you are not neutral yet.

Pick between fresh and dry-woody with your eyes, not your nose
These two are close enough that the interior decides
Fresh and dry-woody are both correct for a clinic and the difference between them is not really a quality difference.
